According to the Centers for Medicare & Medicaid Services (CMS), accountable care organizations, known as ACOs, are groups of doctors, hospitals, and other health care providers, who come together voluntarily to give coordinated high-quality care to their Medicare patients. Being a member of an ACO allows for coordinated medical care across multiple providers. It reduces time between accessing services and allows for your providers to communicate treatment plans more effectively. If you are a member of an ACO, you may be working with a dedicated case manager to support you in accessing services.
Elite Home Health & Hospice proudly supports ACO members in accessing home health services. We appreciate the level of coordination and engage in weekly reviews of patients, their progress with therapies and nursing, and identify needs.
If you have questions about accessing home health through your ACO benefit, send us an email at info@EliteHHH.com
