Clinical Focus
It is difficult to go anywhere or do anything without considering COVID. Through our unique role in the community as healthcare providers and clinicians providing essential home health and hospice services, we are doing our part to support safe practices. We continue to abide by recommendations for PPE use and infection control regulations. Clinicians are required to wear face masks, eyewear, and gloves on their visits. Regular use of alcohol based hand sanitizers are integral given limited access to handwashing stations while on the road. We continue to practice standard infection precautions, such as proper bag technique and disinfection of contaminated equipment between visits. Question Corner
Elite Home Health & Hospice is focused on timely initiation of care. As such, we are focused on building a complete referral from the start. A complete home health referral has:
1) A demographics page
2) Current H&P
3) A current medication list
4) A PCP who will follow the course of treatment and sign orders
Please mention to your patients if you are recommending home health services. We want to make sure that the services you ordered are able to be provided. Building a patient’s understanding of your recommendations is a critical piece in collaborating with you and the patient on their care plan.
