Clinical Focus

We have another packed month with opportunities to honor our amazing staff and highlight strategies to support conditions that impact our community. This week we want to recognize our nurses. Our nurses support patients in their home health services through focused education and disease management. They provide wound care and case management. These RNs are uniquely trained and specialize in meeting the ever-changing needs of their patients and the team members that are involved in the plan of care. Our nurses also direct all the provisions of our hospice program. They complete the assessments to qualify and establish hospice services. They coordinate the members of the team to identify the unique needs of the patient and their family. They serve as a point of contact in our 24-hour nurse line to aid in patient questions and triage. Our nurses are fundamental to everything we do as a home health and hospice agency!

What is skilled nursing service on home health?

Skilled nursing is often ordered on home health patients. Obvious needs for wound care demonstrate the unique skills and training of our nurses.  However, disease process training and management is an essential component of the skilled nursing intervention. Nurses spend time with patients helping them to understand the value of health behaviors, such as keeping logs of vital measures (e.g., weight, SpO2, BP, etc.) to support physicians in the care and management. Nurses train medication regimens, side effects, and timely administration to support accuracy and achieve desired outcomes from prescriptions. Patient adherence to these interventions is supported through training and the use of tools to aid ongoing practice after services are completed. In addition, nurses often identify obstacles to patient’s success and incorporate other professional in therapy to aid patients in their recovery goals.
